50:50 contribution flexibility form 

Complete this form to opt for the 50:50 section or to return to the main section of the scheme.

Going into retirement


Retirement action form 

Active councillor members
You only need to complete this form immediately after leaving your role, if you wish to claim retirement benefits from your active membership. Your authority should provide this form to you at the appropriate time.

Deferred councillor members
If you're no longer in your role and have deferred your pension, complete this form when you're ready to claim your deferred retirement benefits.

If you leave the LGPS


Member leaver form 

Complete this form if you have left your role and you wish to do one of the following:

    • Receive a refund (subject to eligibility).
    • You are aged 55 or over and wish to investigate receiving your pension or you wish to transfer out your pension rights to another scheme.

Opting out of the LGPS


Opt out form 

If you wish to opt out of the pension scheme, you need to complete this form and send it to your payroll provider.

Opting out is an important decision as you will lose out on valuable benefits - therefore we recommend you carefully read the form before taking this course of action.
